Although we are a dental practice, we are also concerned about your total health. Before any diagnosis or treatment can be taken, we ask that you to complete a health questionnaire and bring it with you to your first appointment. The information you provide will help us treat you safely and comfortably. The health history will be updated during routine follow-up visits.

Insurance Plans
We are not a participating provider with any insurance plan but we will gladly submit your claim. Please bring the necessary information with you on your first visit.
Insurance policies generally cover only a portion of the total treatment cost. Unless other arrangements have been made, we ask that you pay your portion of the bill at the time of treatment. It is your responsibility to pay any balance not paid by your insurance company.
